In this role, you will be primarily responsible for conducting comprehensive medication reconciliation for patients across care transitions in inpatient settings. You will review and verify patients’ medication histories, identify and resolve discrepancies, and ensure medication lists are accurate and complete to prevent errors. Collaborating closely with physicians, nurses, and other healthcare providers, you will help optimize medication regimens, reduce adverse drug events, and improve clinical outcomes. Your work will contribute to seamless transitions of care and enhanced patient safety.
You will also provide clinical pharmacy services related to medication reconciliation as per departmental policies, including recommending dose adjustments, therapeutic alternatives, and ensuring adherence to medication protocols.

Hourly Salary Range: $90.16 - $105.61 (Union represented, step-based salary depending on years of experience)
