Description: The Pulmonary Educator is responsible to complete patient assessments on designated inpatients and Pulmonary Rehabilitation patients with common pulmonary conditions. This assessment will include an evaluation of the patient’s understanding of disease processes, medication management, ongoing therapies, treatment plan, and adherence to therapy. Identify patients that are at high risk for readmission.
