Job Description Summary The Social Worker (MSW) I reports to the Case Management/Social Work Supervisor. Under guidance of supervisor, the Social Worker I (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge.
