Description This is an on-call/per diem position supporting PACE sites in Seattle and Kent, WA. The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. Description The physical therapist (PT) delivers and supervises others in the delivery of physical therapy services. The PT performs evaluations and develops individual plans of care to maximize functional outcomes, implements the plans of care and reassesses treatment results, and actively participates in discharge planning. Services are delivered within the framework of a team approach, including appropriate disciplines, significant others and the patient. Provision of care for pediatric adolescent orthopedic patients may be required. Coverage of both inpatient and outpatient services may be required at the Ballard and Issaquah clinics.
