Summary:
UNC Health is hiring two Community Health Consultants / Community and County Engagement Liaisons to support NC ROOTS across Region 4. Each liaison will serve as the primary ROOTS contact for approximately two to three assigned counties, building trusted relationships with local leaders, healthcare providers, community-based organizations, nonprofits, faith communities, and residents.
Working under the direction of the ROOTS Executive Director, the Consultants will help advance the broader community health strategy by identifying and elevating local priorities, barriers, and opportunities; connecting community partners with relevant ROOTS initiatives and resources; and supporting the planning, implementation, and evaluation of community health programs. This work will include initiatives focused on health equity and social determinants of health, including healthcare access, transportation, health literacy, and food insecurity, as well as support for community outreach and social marketing efforts.
The Consultants will play an important role in ensuring ROOTS strategies and programming reflect the needs of the communities they serve. Ideal candidates will bring strong community engagement and relationship-building experience, preferably within the counties they will support, and be comfortable traveling regularly throughout Region 4 and attending occasional evening community meetings.
Responsibilities:
Community Engagement - works with coalitions and various groups to determine opportunities to collaborate on work utilizing the collective impact framework to assess community needs, assets, capacity, and partnerships to carry out community health programming.
Relationship Building – Functions in a way that builds trust internally within the health care system and externally with patients and community partners respectfully valuing input from all partners. Demonstrating Self-awareness and practicing open communication.
Cultural Competency - having knowledge about individuals and groups and translating to specific standards, policies, practices, and attitudes used in appropriate cultural settings to increase the quality of care; thereby producing better outcomes.
Program Planning & Evaluation – conducting research assessing needs, setting goals and objectives, planning activities, implementation and measuring outcomes.

● Minimum three (3) years’ of experience in community engagement which should include: •Minimum one (1) years’ experience initiating and developing projects/programs/initiatives in a complex organization •Minimum one (1) years’ of training and facilitation experience •Minimum one (1) years’ experience working in healthcare industry
