Summary:

The University of North Carolina (UNC) Medical Center Department of Pharmacy has an opening for a highly motivated pharmacist seeking to join our team of clinical pharmacists in Chapel Hill, NC. The focus of this position will be on providing comprehensive pharmacist care services and disease management of patients seen through the adult oncology service line. This individual will ensure continuity of care as the patient transitions between acute care and ambulatory care settings, while continuing to pursue best practices in this area. The services covered will primarily consist of specialized pharmacy services in the medical oncology service space. This position will ensure that clinical coverage can be provided during absences within the team. Additionally, a responsibility exists for providing experiential instruction to Doctor of Pharmacy Students and UNC Medical Center pharmacy residents.

This individual will directly report to the Clinical Manager of the Hematology/Oncology Service Line. Additionally, this individual will work closely with the Hematology/Oncology Clinical and Central Specialists as well as the Specialty Pharmacists within the UNC Carolina Care Pharmacy Care Network

RESPONSIBILITIES

The primary job responsibilities include providing comprehensive pharmacist care services and optimizing medication use for patients seen through the medical oncology service through innovative clinical services, ensuring continuity of care by bridging acute and ambulatory pharmacy services, and leading quality improvement initiatives. The Clinical Pharmacist Float will provide coverage in the inpatient or outpatient services to support absences in staffing due to time off, administrative days, or vacancies.

Clinical Responsibilities

• Medication therapy management
• Provide medication therapy management for inpatient service lines and outpatient clinics.
• Serve as a liaison between the Department of Pharmacy and the Division of Hematology/Oncology to assure all activities are coordinated and consistent
• Educate patients and their caregivers about their chemotherapy plan
• Support medication management goals set forth by the institution or Department of Pharmacy
• Manage chemotherapy toxicities
• Function as a limited oncology prescribing practitioner (LOPP) to collaboratively manage treatment plans within Epic electronic health record (EHR)
• Coordinate medication related aspects of transitional care
• Collaborate with the acute care pharmacists and cancer hospital infusion pharmacy team caring for the hematology/oncology patients on pharmacy related policies, procedures, and guidelines
• Respond to drug information questions from the medical staff, nursing staff, or pharmacy colleagues · Evaluate, monitor, and redesign medication regimens to ensure positive patient outcomes · Provide comprehensive medication management for patients receiving oral chemotherapy agents· Specialty Pharmacy Services:· Serve as a clinical liaison among the clinical oncology team, UNC Medication Assistance Program, and specialty pharmacies (including UNC Shared Services Center Pharmacy) to help address medication access barriers and ensure timely processing and delivery of specialty medications

Service Responsibilities·

• Serve as a standing committee member on a select group of committees within UNC Medical Center
• Contribute to ongoing quality assurance initiatives for the Department of Pharmacy, the Hematology/Oncology Division, and UNCMC

• Educational Responsibilities

• Educate students from the UNC Eshelman School of Pharmacy, PGY1, and PGY2 residents from UNC Hospitals on their experiential rotations

Education Requirements:
● PharmD or BS Pharmacy Degree
Licensure/Certification Requirements:
● Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.

Knowledge/Skills/and Abilities Requirements:

• • Extensive clinical skills and pharmacotherapy knowledge base in Oncology
  • Strong oral and written communication, organizational, and prioritization skills
  • Ability to work in team environment
  • Ability to generate meaningful contributions related to oncology and advancement of pharmacy practice.
  • Ability to design and implement projects and collect and interpret data accurately

Qualifications and Required Experience:

Required:· Doctor of Pharmacy Degree from an accredited college or university or equivalent degree · Licensure or license-eligible as a pharmacist in the State of North Carolina Preferred:· Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience · Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience · Board Certification in Oncology Pharmacy, Board Certification in Pharmacotherapy, or equivalent certification Job Details
