As part of the region’s largest health system, you will join a collaborative home health therapy team dedicated to delivering coordinated, high-quality care across the Central Valley through an integrated network of hospitals and outpatient services. The Home Health team delivers patient-centered rehabilitation services in the home setting, supporting patients in regaining function and independence where they live. In this role, you will serve as an essential member of the interdisciplinary care team, providing skilled occupational therapy services tailored to each patient’s goals, environment, and cultural context.
You will evaluate patients in their homes, develop individualized occupational therapy plans and goals, and communicate findings and progress with patients, families, physicians and the care team. You’ll provide or supervise home-based treatment, document outcomes according to Community Medical Centers standards and determine when services should be concluded. You will also educate staff and students, support team coverage as needed, and adapt care to each patient’s home environment and needs.
