Ambulatory Pharmacist - UVA Health Outpatient Retail Pharmacy

Job Description

UOVUNIUSR0085255EXTERNALENUS

As an Ambulatory Pharmacist with UVA Health Outpatient Retail Pharmacy, you will play a critical role in advancing patient care beyond traditional dispensing by integrating clinical expertise, direct patient engagement, and collaborative decision-making. This position offers the opportunity to work within a growing ambulatory pharmacy program where pharmacists are empowered to make meaningful impacts on patient outcomes across multiple care settings.

In this highly visible role, you will care for recently discharged patients, specialty clinic populations, UVA Health employees, and community members while leveraging full access to the electronic medical record. You will practice at the top of your license, participate in the expansion of pharmacist-led clinical services, and collaborate closely with physicians, nurses, care teams, and pharmacy colleagues to deliver exceptional medication management and patient-centered care.

The Role

This is a dynamic, rotating-shift ambulatory pharmacist position within UVA Health's outpatient retail pharmacy network. Unlike a traditional retail environment, you will have comprehensive access to patient medical records, allowing for deeper clinical assessment and more informed medication decisions. You will rotate through multiple service areas, supporting patient care transitions, medication access, immunization programs, and community pharmacy services while serving as a trusted clinical resource for both patients and providers.

An average day may include:

  • Reviewing prescriptions for clinical appropriateness, safety, and efficacy

  • Utilizing EMR data including diagnoses, allergies, medication histories, laboratory values, and provider notes to support clinical decision-making

  • Collaborating directly with physicians and care teams to resolve medication-related concerns

  • Counseling patients on medications, treatment plans, and disease-state management

  • Verifying medication products prior to dispensing

  • Identifying and resolving medication access barriers

  • Supporting discharged patients through medication reconciliation and transitions of care

  • Coordinating prescription fulfillment and patient education services

  • Conducting vaccine screenings and administering immunizations in dedicated clinical spaces

  • Rotating through multiple pharmacy workflow assignments to ensure comprehensive operational and clinical support

This role offers exposure to a broad spectrum of ambulatory pharmacy practice, including:

  • Discharge medication services and transitions of care

  • Community and employee pharmacy services

  • Immunization and preventative health programs

  • Medication Therapy Management (MTM)

  • Naloxone/Narcan services

  • Smoking cessation programs

  • Contraception initiation services

  • Future clinical service line expansion opportunities

The practice environment is collaborative, fast-paced, and professionally rewarding. Pharmacists work alongside a highly experienced team of pharmacists and pharmacy technicians while maintaining individual accountability and autonomy in clinical decision-making.

Work schedules rotate and may include shifts beginning as early as 7:00 AM and as late as 12:30 PM to 9:00 PM. Weekend and holiday rotations are required. Pharmacists may also support other outpatient pharmacy locations across the health system when operational needs arise, including Zion Crossroads, Emily Couric Cancer Center, Student Health, Bookstore Pharmacy, and other UVA Health retail locations.
