Summary:
Assists the Director in organizing departmental activities, evaluating case management process, evaluating personnel, and implementing the Orientation and Competency Program. Coordinates & provides case management to an assigned unit based on staffing needs at least 50 % of his or her scheduled time. Serves as clinical resource for the patient, staff, and other clinical departments. Assists nursing staff with priority setting.
Responsibilities:
1. Demonstrates clinical expertise in execution of the nursing process for case management. Functions as a role model in the specialty. Provides direct case management! utilization review as needed. Functions within RN scope of practice. Adheres to Case Manager's job description. Serves as a resource person for departmental staff. Provides direction to staff as requested and evaluates care provided by departmental staff.
2. Assures Case Managers participates in initiating the interdisciplinary plan of care. Collaborates with the Care nursing and other disciplines regarding patient I family education and discharge planning needs.
3. Assist the department staff in monitors the hospital length of stay and collaborating with the medical and ancillary staff for an aggressive discharge plan. Performs proficiently, remains calm, communicates appropriately, and documents situations.
4. Maintains competence in assigned clinical setting. Fulfill all expectations of the Case Manager duties to include maintaining set of competencies for given specialty area.
Participates in the interview, selection, recruitment and hiring of nursing employees as directed by the Director. Directs efforts at maintaining staff minimizing staff turnover.
5. Effectively communicates the philosophy and policies of Nash Hospitals, Inc., and the department to employees. Demonstrates effective leadership skills in delegation, organization, and coordination. Demonstrates an awareness of self-responsibility and accountability for own professional practice and continuous professional development. Serves on committees, work groups on a voluntary and appointed basis at the unit, department, and hospital level.
Education Requirements:
• Graduate of an approved nursing program.
Licensure/Certification Requirements:
• Current License to practice as a Registered Nurse in North Carolina.
• BCLS Certification.
• ACLS Certification.
Professional Experience Requirements:
• 3 years of experience Healthcare/Medical - Nurse: Registered Nurse.
