This role supports UNC Health’s Memory Connect program—a virtual, interdisciplinary, primary care–integrated dementia care model—by providing comprehensive social work services to people living with dementia and their caregivers. As a key member of a collaborative team that includes an APP and a Geriatrics physician, the Social Worker (BSW) delivers personalized care planning, caregiver education, behavioral symptom support, and coordination with internal and community resources. The position leverages standardized Memory Connect protocols, EHR-based templates, and shared care plans to ensure seamless communication across care settings, while contributing to ongoing quality improvement efforts as the program expands systemwide beginning in 2026.
Summary:
As a Social Worker BSW at UNC Health, you will play a vital role in providing support, resources, and advocacy for patients and their families. You will be responsible for facilitating resources, advocating for patients' rights, actively listening to their concerns, demonstrating respect for individuality, building trust, and obtaining, interpreting, and communicating necessary information regarding patients and families, including bio-psychosocial history information.

● Bachelor's degree in Social Work (BSW) from a program accredited by the Council on Social Work Education.
Licensure/Certification Requirements:
