Loan forgiveness and tuition reimbursement programs are available.  RESPONSIBILITIES Coordinate patient discharge planning and develop safe, individualized transition plans in collaboration with physicians, patients, families, and community partners. Serve as a patient advocate by connecting patients with appropriate resources, facilitating care coordination, and supporting patients and families during hospitalization and discharge. Complete discharge assessments, maintain accurate documentation, provide post-discharge follow-up, and coordinate referrals based on patient needs. Partner with Utilization Review, Patient Financial Services, and healthcare providers to support appropriate levels of care, insurance coordination, and complex case management. Participate in interdisciplinary meetings, maintain current community resource information, monitor quality indicators, and contribute to continuous quality improvement initiatives. KNOWLEDGE AND EXPERIENCE Current Alaska Licensed Clinical Social Worker (LCSW) required, or other qualifying social work licensure and experience as outlined for the position level. Minimum of one year of hospital-based discharge planning or case management experience required; additional experience may be required depending on licensure level. Working knowledge of discharge planning, care coordination, Medicare, Medicaid, VA benefits, private insurance, and community-based support services. Strong communication, collaboration, advocacy, and crisis intervention skills with the ability to work effectively across multidisciplinary teams. Ability to maintain accurate documentation, prioritize multiple responsibilities, and exercise sound clinical judgment in a fast-paced healthcare environment. Commitment to professional development through continuing education, required training, and active participation in interdisciplinary teams. Physical Requirements: Ability to sit, stand, and walk throughout the workday, with occasional lifting of up to 25 pounds. ENVIRONMENT The South Peninsula Hospital is a combined 22-bed Acute Care Critical Access Hospital and 28-bed Long Term Care facility located at the southern end of the Kenai Peninsula in South-Central Alaska. This position works primarily indoors in a fast-paced, patient-centered hospital environment where collaboration, organization, and attention to detail are essential. Work schedules may vary based on operational needs and require frequent interaction with patients, families, providers, and community partners.
