Summary
Assists families struggling with social challenges to connect with appropriate resources and overcome barriers to care. Helps meet the basic human needs of patients, with particular attention to the needs and empowerment of people who are vulnerable, oppressed, and living in poverty. Working collaboratively across the service line, and through assessment, referrals, and interventions, enhances the capacity of patients and families to address their own needs. Serves as an advocate for patients, participates in multidisciplinary care coordination, assists with crisis intervention, and mentors other professionals in support of families. Aligns efforts towards the achievement of service line goals.
Education
Master’s Degree in Social Work.
Licensure/Certification
Must have current Indiana LSW license.
Experience
Minimum two years’ experience working with families with social complexities and cultural barriers to healthcare.
Other Qualifications
Displays maturity and models healthy boundary setting. Shows discernment of own self-care needs and proactively engages in activities to support own mental health. Has robust understanding of community resources in Northeastern Indiana region. Displays cultural competence.
