Infectious Disease - Ambulatory Care
The Ambulatory Care Clinical Pharmacist will optimize drug therapy outcomes by providing high quality, cost-effective health care; influencing prescribing behaviors; identifying, preventing and resolving drug-related problems; and educating health care personnel regarding drug therapy.
Participate in clinical programs to improve patient care, including but not limited to participating in in-person or telephonic pharmaceutical care clinics, therapeutic drug monitoring programs, participate in consult agreements, and programs to support state-wide initiatives.
Current practice for this role involves but is not limited to:
Managing OPAT patients; responsibilities include completing a referral note within 48 hours of hospital discharge, monitoring weekly (or 2x weekly) labs, managing side effects, aiding in transitions of care concerns, following clinical progress, and determining plan of care at end of therapy
Rounds with each RN/MD team to review OPAT list
Aid in HIV and PrEP counseling and medication management (including prior auths and insurance concerns where needed)
Aid in TDM for long term antifungals
Organize and facilitate PGY1 ID rotation for The Jewish Hospital – Mercy Health’s Residency Program
Answer any drug information questions from MDs, RNs, inpatient pharmacists etc.
Education:
· Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD (required)
· Advanced pharmacy degree – Doctor of Pharmacy, Masters, Residency (preferred)
Required Licensing & Certifications:
· Maintains current license for pharmacist in the state(s) where practicing (required)
· Valid state issued ID (required) ·
· Immunization Certified (preferred)
Experience:
· 2 years of recent experience in an acute care setting (required)
· 1 year of leadership experience (preferred)
