Summary

Evaluates patient’s condition and/or disability, by selecting and administering appropriate tests and measures; formulates and administers a treatment plan; interprets testing and treatment to staff, patient, family; determines need for referral to other specialists and/or agencies; maintains proper records and reports. |

Must have a Master’s degree in Speech-Language Pathology. |

Must hold the Certificate of Clinical Competence (C.C.C.) from the American Speech-Language-Hearing Association (ASHA). Must hold or be eligible to receive a Speech-Language Pathology license in the State of Indiana, from the Indiana Professional Licensure Board. Must have a valid state driver's license, reliable transportation, and proof of insurance. Must have current CPR certification. |

At least one year experience in a clinical setting related to the departmental scope of service provided. In lieu of previous work experience, verification of supervised clinical experience in evaluation and treatment of communication disorders. |

Must be willing to travel within the workday and be willing to make home visits. Must be able to lift 50 lbs. |
