A Brief Overview
The Breast Health Navigator is responsible for coordinating patient clinical and financial services. This includes aspects of treatment, evaluation, support services and education to patients. The navigator coordinates clinical documentation from referring and treating physicians for each patient. The Breast Health Navigator is a member of the Radiation Therapy Center, and partners with Case Managers on patients who require a wider dimension of services and are part of the collaborative efforts of a multi-disciplinary team to achieve specific outcomes and objectives over the continuum of care. Overall, the Breast Heath Navigator facilitates the guidance of individuals through the healthcare system increasing their understanding of their illness, treatment, pharmacy needs, financial issues, home health care, rehabilitation, etc. A valid South Carolina driver's license and own personal transportation required.

What you will do
  • Answer all patient calls (24 hours a day, seven days a week).
  • Meet with all newly diagnosed breast cancer patients and review prepared educational materials, pre and post op instructions, and appointments to ensure a continuum of care.
  • Schedule all metastatic workups for patients.
  • Assist patients with financial, transportation and emotional needs by connecting them with other sources of support while maintaining their independence in decision-making.
  • Provide post-surgical camisoles to Breast Cervical Screening and mastectomy patients.
  • Prepares all charts and materials for Breast Tumor Board and forwards a list of these patients to the Cancer Registry.
  • Maintain breast cancer log which includes all diagnosed patients.
  • Maintain and coordinate Mammosite Registry Trial follow-ups and cataloged all forms.
  • Mail educational materials to all benign biopsy patients and serve as a source of education about community resources for other facilities and patients.
  • Instruct self-breast exams to groups and/or individuals.

Education Qualifications
  • Associate's Degree in related Healthcare field Required
  • Bachelor's Degree in related Healthcare field Preferred

Experience Qualifications
  • A minimum of (3) three years’ experience in healthcare delivery in the community setting.
  • A minimum of (2) two years specific experience with hospital and other agency and provider reimbursement procedures policy and practice
  • • A minimum of (2) years experience with community resources referral and management to assist patient access to health care interventions and navigation through bureaucracies of a clinical system.

Skills and Abilities
  • Demonstration of proficient communication skills, both oral and written.
  • Demonstration of basic PC skills and knowledge of Windows and Microsoft office programs.
