Summary:
Implements high quality/low cost performance improvement initiatives for UNC Health Nash. Works collaboratively with multiple internal stakeholders and departments, including QI, IT and operations, to design, streamline, and implement population health programs across the region. The Population Health Clinical Coordinator will work in close cooperation with community providers and partners in efforts to improve health outcomes and access across the region. Coordinates efforts to achieve and maintain compliance with population health programs and quality metrics.
Responsibilities:
1. Works collaboratively with senior leadership, multiple stakeholders and departments, including QI, IT and operations, to design and operationalize population health programs across the region.
2. Participates in organizational and system committees related to high quality/low cost QI initiatives and population health initiatives. Demonstrates effective communication and presentation skills, identifies common goals and fosters a cohesive team environment across transitions of care service lines.
3. Demonstrates proficiency to work in multiple databases, develop reports and perform data analysis. Uses Epic knowledge in the Inpatient and Ambulatory environment to support productivity and streamline processes across transitions of care service lines.
4. Demonstrates knowledge and provides leadership for healthcare quality improvement initiatives supporting population management, medical home, and quality patient outcome goals.
5. Serves as a project lead for quality improvement work; provide training, support resources, process implementation and oversight, task prioritization, and accountability for hitting productivity targets.

Other information:
Education Requirements:
· Bachelor's degree in related field (i.e. nursing, public health, health care administration, industrial engineering, epidemiology) required.
· Master’s degree preferred.
License / Certification Requirements:
· Clinical Licensure (RN) is required.
Professional Experience Requirements:
· Three (3) years of clinical experience in a healthcare setting required.
· Minimum of 2 years healthcare experience and participation in successful population health programs required. Data management, QI process design, re-engineering, policy development and training.
