Right now we have an opportunity for a Case Manager to join our team in the Emergency Department!

The case manager provides professional services to patients and families, which facilitates the use of individual, public, and private resources so that optimal functioning can occur for both the patient and the family accessing care in the emergency department. These core services include assessment of psychosocial functioning and capacities, counseling for care planning and decision-making, and community resource planning. Mental health and chemical use assessment, crisis interventions, traumas, social admission preventions, and abuse/neglect reporting are prioritized, and case managers will collaborate with many disciplines, programs, departments and organizations in the accomplishment of these priorities.

Required Education/Training:

  • Minimum of a bachelor’s degree in social work, nursing or related field. 
  • Master’s Degree in Social Work preferred.
  • Two years of experience as a Social Worker in a hospital, health organization or mental health care agency.
