Job Summary

We are seeking a dynamic inpatient rehabilitation Admission Liaison to join our team!

Job duties of the Admissions Liaison are: Assesses patients to determine the appropriate level of post-acute care services within CMS/payor guidelines and reviews the options with the patient and/or designated representatives. Primary assignment includes one level of post-acute care (SNF or IRF). Gathers and communicates significant referral information to appropriate staff within the continuum to promote continuity of care post-acute discharge. Responsible for all referral data collection as required. Responsible for supporting the referral development goals for the post-acute continuum and to educate/liaison with physicians, physician office staff, APRN, PA, nursing, case managers and social workers to facilitate the patient's discharge process. Responsible for growing the post-acute business and developing positive and effective relationships within the community, facilities, and other liaisons.


Required Qualifications

- Require current State of Florida licensure in a clinical professional healthcare field (i.e., RN, PT, OT SLP, LCSW).
- Require a minimum of one (1) year of admissions or case management experience or successful completion of SMHCS internship & three (3) years of clinical experience in related discipline or case management experience in a hospital or skilled nursing or inpatient rehabilitation facility setting.
- Require an associate degree in relevant field of discipline, bachelor's or master's degree preferred.


Preferred Qualifications

- Prefer one of the following certifications: - Uniform Data Systems (UDS) - Certified Case Manager (CCM) - Accredited Case Manager (ACM) - Certified Rehabilitation Registered Nurse (CRRN)
- Prefer demonstrated knowledge of commonly used computer applications (i.e., Electronic Health Records (EHR), Windows, Word, PowerPoint, Excel).
- Prefer demonstrated people and customer service skills.
- Prefer demonstrated organization, communication, and presentation skills.
- Prefer demonstrated knowledge in insurance coverage and various covered days by CMS requirements.
