Position Details:
The clinical pharmacy specialist in ambulatory care will provide direct patient care to patients in the ambulatory setting, ensuring safe, cost-effective medication therapy and optimizing pharmacotherapy plans through disease state management programs and collaborative drug therapy management (CDTM) protocols. This position is based in Parkview’s PPG Family Medicine office located in Warsaw, Indiana. The ambulatory pharmacist provides management of diabetes, hypertension, dyslipidemia, asthma, COPD, migraines, PrEP, osteoporosis, and weight management. Parkview Ambulatory Pharmacists practice at the top of their education.
