Description The Intensive Case Manager provides services to outpatient psychiatric clients. They will work with the Community Care Center and home health team to identify and support vulnerable community members. Case management includes providing short term intensive care coordination with partner agencies, housing resources and health care providers. It also includes continued mental health stabilization, assisting clients' success for a given period of time in their community.
