Department Pharmacy Job Summary This role is a mixed position consisting of both inpatient clinical staffing and transitions of care responsibilities. Our transitions of care team focuses on reducing hospital readmissions through medication reconciliation, discharge medication education, identifying and resolving medication access barriers, and supporting safe and effective medication use following discharge, with a particular focus on patients with heart failure and other chronic disease states. Inpatient clinical staffing responsibilities will include a mix of medical/surgical patient care areas. Department Pharmacy Job Summary Responsible for the provision of comprehensive pharmaceutical care in a patient focused manner that supports and promotes medication safety. Directly supervises technical staff. Assignments to be determined based on the specific training and skill set (i.e. unit-based/patient care; satellite services) of an individual employee. Orients and trains pharmacists, technicians, residents, interns and students as assigned. Required Qualifications - Require active Florida Pharmacy license.
