Description The Clinical Behavioral Health Specialist is an integral part of a behavioral health consult team serving acute patients remotely through an innovative Telehealth platform. This position requires independent clinical decision making, flexibility and an ability to provide a variety of assessment and interventions to a wide range of patient populations in multiple geographic regions. Description Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre-admission to immediate post-discharge. Acts in a non-caregiver capacity by facilitating coordination and communication between all member of the health care team, patient and family in the decision making process to minimize fragmentation of the health care delivery system.
