Description This is a per diem/on call inpatient position supporting patients in Santa Rosa Hospital but will be supporting Healdsburg and Petaluma Valley as needed. This position provides psychosocial assessment, delivery of short-term counseling and emotional support, goals of care conversation and advance care planning to patients with serious or life limiting illness and their families.
