Description: The community social worker along with the community case managers identifies patient’s and family’s psychosocial and environmental needs related to health care. The community social worker connects patients and their families to the appropriate community resources to successfully maintain or improve their desired level of care. The community social worker will meet with patients and families in their homes to assist with problem solving, application paperwork and determining needed resources.
