Description The Physical Therapist (PT) is an important member of our interdisciplinary team, providing direct clinical services within Swedish Pain Services (SPS). This includes working in both our outpatient Functional Restoration Center and our hospital-based Pain Management Suite. The SPS Functional Restoration Center offers comprehensive outpatient pain management through an interdisciplinary approach. Our services include behavioral health, physical and occupational therapy, and structured Functional Restoration Programs (SFRP). Description The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.
