41342770 Overview In a per diem capacity; acts as a pharmacist in training. Develops advanced practice skills by providing comprehensive pharmaceutical care through integrated drug distribution, clinical services and teaching. Responsibilities Provides pharmaceutical care by assisting physicians in various tasks, including, but not limited to, reviewing patient medication history, educating patients on drug therapy, documenting medication related issues in patient chart, etc. Makes patient care rounds. Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. Provides pharmacokinetics consultations.
