UOVUNIUSR0082031EXTERNALENUS UVA Physicians Group is seeking an Ambulatory Care Clinical Pharmacist for two of our primary care practice locations, UVA Primary Care Culpeper in Culpeper, VA and UVA Primary Care Locust Grove in Locust Grove, VA. The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting.  The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to:  designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting.
