General Statement of duties:
Patient outreach and education (medication, infusion equipment)
Discharge planning coordination with Hospital and Home Health staff
Care planning and clinical review for I.V., enteral nutrition & hospice patients
Multidisciplinary problem solving with clinical and logistical coordination
Sterile drug compounding and supervision
Drug utilization review (clinical and payer related)
Education: Minimum of a B. S. degree in Pharmacy from an accredited school of Pharmacy. A...
