Description This position will be supporting patients in Jackson and Josephine counties. The Licensed Clinical Social Worker (LCSW) independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems. This position works closely with the hospice interdisciplinary team to provide clinical counseling, assessment, case management, crisis intervention, and psycho-education with multi-stressed patients and families relevant to the dying process, end of life care planning and bereavement.
