Job Summary

This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. This position contributes to a collaborative, patient-centered environment and helps drive outcomes aligned with organizational priorities.

The Admission Coordinator will work with the hospital, home health, other facilities (hospitals, ALF, SNF) and hospice team to coordinate care across the continuum. The hours are mostly daylight with occasional evenings and weekends as required to meet patient and family needs.

Essential Job Duties

  • Possesses a working knowledge of nursing principles
    • Is clinically competent. Is consistently able to make appropriate decisions.
    • Displays initiative and responsibility for professional growth.
    • Knowledge of hospice
    • Make suggestions to enhance the quality of work and follow through enabling professional growth.
    • Consults with hospital staff regarding current and prospective hospice patients.
    • Receives referrals from hospital staff or physicians. Communicates and collaborates with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
    • Informs patient and support system of hospice benefits and responsibilities of the patient and caregiver.
    • Assesses patient for appropriateness of referral to hospice. Cares for patients across the life spectrum.
    • Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify the physical, psychosocial, and environmental needs.
    • Conveys accurate information and relays it in a timely fashion to the hospice team and hospital team involved in the patients care.
    • Willingly shares clinical knowledge to the good of the patient.
    • Enters referral information via Intake process.
      • Serves as backup for Hospice Intake coverage as needed.
    • Accurately receives information and relays it in a timely fashion.
    • Provides effective communication to patients, their family members, team members, and other health care professionals.
    • Coordinates financial assistance for patients in need with hospital social workers.
    • Completes documentation in patient’s hospital record of all interactions.
      • The information is accurate and appropriate.
    • Recommends appropriate patient placement on discharge.
    • Assists with coordination of care, hospice DME, orders, and medications when transitioning to hospice from the acute hospital stay/ED visit.
    • Educates hospital staff regarding the benefits of hospice.
    • Timely submission of all required paperwork per policy.
    • Provides care utilizing infection control measures (OSHA).
    • Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency. Participates in the Hospice’s QAPI program.
    • Hospice care management for hospice patients in the hospital and, in the home, including the Shenandoah House as needed to meet patient needs.
    • Promotes the Hospice philosophy.

Required Qualifications

  • Education: Diploma or Associate Degree in Nursing
  • Licensure/Certification: Virginia State RN License
  • Experience:
    • 2 years RN experience in clinical care
    • 1-year current RN experience in hospice
  • Driver’s License: Yes
    • To drive an Augusta Health Vehicle, must be 21 years old and Motor Vehicle Record (MVR) must be approved by Risk Management.
  • Eligibility to work in the United States and meet Virginia state employment requirements

Preferred Qualifications

  • Education: Bachelor's Degree in Nursing
  • Licensure/Certification: Certified Hospice and Palliative Nurse (CHPN)
  • Experience: None
  • Prior experience in a hospital, healthcare system, or related service-oriented environment
  • Familiarity with Augusta Health’s systems, workflows, or organizational culture is a plus
