Job Overview:
The Food Access Coordinator will conduct the day-to-day operations of Jersey City Medical Center s Green Apple Rx, JCMC Food FARMacy. This employee will enroll participants, manage food pantry distributions and help to meet grant deliverable requirements for the program. The employee will work with our partners to order, receive, stock, and distribute food pantry inventory. We are looking for an enthusiastic team member who is passionate about making healthy food available for our community
Qualifications:
Required:
High School Diploma Required
Individuals with a bachelor s degree in Nutrition and Dietetics or Public Health are best suited for this role
Must possess and maintain a valid NJ State Driver s License
Comprehension of food security/food access issues
Demonstrates the ability to function as a member of an interdisciplinary team
Experience working with urban populations of multiple ethnic and cultural backgrounds
Good oral and written communication skills
Proficiency in Microsoft Office
Experience in EPIC or other electronic medical record system
The ideal candidate for this position has excellent organizational, interpersonal and empathetic skills
Preferred:
Bilingual strongly preferred

Essential Functions:
Responsible for supporting the daily operations of the Greenville Food Farmacy by coordinating or carrying out cleaning, arranging machine maintenance, and/or moving products from storage to main pantry
Managing food pantry distribution hours: packing client bags and supervising appropriate dispersal of inventory
Maintains up-to-date records of all patient encounters and documents accurately in the chart to ensure demographic information is accurate for monthly reporting
Ordering, receiving, stocking, and maintaining food pantry inventory following food safety protocols
Registering Food Farmacy patients who are referred by their healthcare provider and completing all necessary enrollment paperwork
Conducting in-person or phone pre and post program intervention surveys with patients
Works closely with the Greenville Primary Care team and Social Impact team
Responds in a timely manner to verbal and written requests from patients, inter-disciplinary team members, administration, medical and public agencies
Assist with community partner and Food Farmacy departmental administrative tasks as assigned
Promote a positive and healthy workplace environment and interacts with other disciplines, departments, agencies and companies in a professional manner
Performs other duties as assigned
