Job Summary:
The Clinical Pharmacy Specialist - Critical Care is responsible for providing comprehensive pharmaceutical care to patients through clinical services, formulary management, development of policies and procedures, and integrated drug distribution. Individual will be responsible for providing recommendations and/or assuming responsibility for critical care patients, including (but not limited to) pharmacokinetics, parenteral nutrition support, anticoagulation or other medication regimens as prescribed by provider.
Core Responsibilities and Essential Functions:
Provide recommendations and/or manage therapy associated with antimicrobial stewardship/pharmacokinetics, anticoagulation, parenteral nutrition, or other medication regimens as prescribed by the provider. a.Document clinical interventions on an ongoing basis. b.Round with interdisciplinary teams, as able c.Demonstrate a basic understanding of distributive responsibilities including computer functions. d.Assist in investigational drug studies when needed. e.Provide on-call clinical pharmacy services as needed. f.Perform antimicrobial stewardship activities as outlined per Wellstar policies. Participate on assigned Committees a.Participate as a member of the Wellstar Pharmacy and Therapeutics (PT) Committee, as well as other System and facility-specific committees as assigned or as deemed necessary. b.Identify target medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations (MUE) or other critical care-related studies/initiatives. Tabulate and present completed MUE data to the PT Committee. c.Monitor metrics related to critical care patients and develop improvement strategies as necessary. d.Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable. e.Assist in preparing for and maintaining compliance with regulatory agency (e.g., The Joint Commission) standards/requirements. Educational responsibilities a.Develop and provide educational programs/inservices for health care practitioners and patients as required. b.Provide drug information when needed or requested. c.Serve as a preceptor/instructor for students from accredited colleges/schools of pharmacy. d.Serve as a preceptor for the Pharmacy Residency Program (where applicable). e.Assist in developing drug protocols.

Board Certification (e.g., BCPS, BCCCP, etc.) Preferred
Georgia Pharmacist License within 180 Days
Required Minimum Experience:
Minimum 3 years years clinical experience or Completion of a PGY-1 Residency Required or
Minimum 2 years years of clinical pharmacy experience in Critical Care or Emergency Medicine or Completion of a PGY2 in Critical Care or Emergency Medicine Required and
Experience with nutrition support, anticoagulation management, and pharmacokinetics Preferred

Strong verbal and written communication skills High
Strong computer skills. Experience with Epic, Microsoft Office, TheraDoc etc. preferred. High
Capable of preparing and providing presentations. High
