Description This is an on-call/per diem position supporting PACE sites in Seattle and Kent, WA. The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA. Description As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum.
