Summary
Primary role is to assess patient needs in order to perform appropriate interventions to support the patient in the orthopedic service line continuum of care. Works collaboratively with various members of the health care team to provide guidance and support to patient to reduce obstacles of care, and to facilitate a clear path back to baseline. Is a creative problem-solver who serves as knowledgeable resource and coordinator between client, healthcare team, and available community resources. Assists patients with obtaining community assistance by referral and coordination with appropriate resources and scheduling and preparation for appointments.
