The registered dietitian (RD) per diem, collaborates with the multidisciplinary team to provide general nutrition care that is integrated with overall goals of care for adult and/or pediatric patients at risk for nutrition problems in inpatient and outpatient settings. The RD in this classification works in inpatient or outpatient pediatric and adult settings to provide nutrition care for patients with a wide range of clinical problems. Primary responsibilities include nutrition assessment, nutrition diagnosis, nutrition intervention and evaluating clinical outcomes; coordination of daily work for dietetic technician(s) (DTR) assigned to area; participation in compliance, department projects, and quality performance improvement efforts; active involvement on interdisciplinary teams and committees; and contributions to the supervision of dietetic interns, and teaching assigned classes.

Description This is a per diem/on call inpatient position supporting patients in Santa Rosa Hospital but will be supporting Healdsburg and Petaluma Valley as needed. This position provides psychosocial assessment, delivery of short-term counseling and emotional support, goals of care conversation and advance care planning to patients with serious or life limiting illness and their families. Works within an interprofessional team to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.
