Job Details: Will be providing LCSW services in Hospice Home Care. Responsible for provision of social work services to include completion of psychosocial assessments and development of plan of care, explanation of services, crisis intervention, resource referral and emotional support and/or counseling to patients and families as needed. Ability to work in coordination with other disciplines at part of team to address end-of-life needs. Masters Degree in Social Work with NC LCSW or LCSW Provisional license required. At least 1 year experience in medical social work and knowledge of end-of-life issues required. Reliable transportation and valid NC Drivers license required.
