Summary Cook Children’s Health Care System will, for the first time ever, have a clinic dedicated to the needs of children with complex medical needs.  The clinic will be a hybrid model and serve as a transitional medical home for those children with medical complexity who need access to a PCP or need more intense support at critical times such as after a hospital admission or acute change in their health status.  The clinic will also serve as a consulting service to PCPs who may need some assistance with managing a complex plan of care for their patients.  Patients can be referred to this clinic by their PCPs, specialists, and other sources outside of our system.  The clinic will provide ancillary support to families; such as access to a social worker, RN care coordinator, and a dietician.  We will use a team-based approach to provide comprehensive and customized care.  The team will work closely with our specialists to ensure that care is coordinated and evidence-based clinical...
