Job Description

Covenant Health is seeking a full time, day shift Social Worker (LCSW, LMFT) with the Mental Health Improvement Department at Covenant Health Partners.

Job Summary:

Maintains accountability and responsibility for efficient, effective delivery of patient care services through appropriate utilization of hospital resources. Responsible for proactive discharge planning in collaboration with the medical staff and ancillary departments within the integrated delivery network for provision of optimal patient care outcomes within the continuum of care. Responsible for negotiation of cost-effective care among clinical team members, third party payers and external agencies providing extended care.

Essential Functions:

  • Evaluation and assessment of situation and functioning to identify individual needs and discharge planning assistance requirements in order to develop a plan with specific objectives, goals, and interventions to address those needs.
  • Implementation of process of coordinating, organizing, securing, integrating, monitoring, and modifying resources necessary to accomplish goals set forth in the plan and evaluate plan's effectiveness.
  • Acceptance and response to referrals from multi disciplinary health care team and community regarding clients who may require social services assistance. Maintains current knowledge of the social welfare system, its programs, and applicability to patient and family needs.
  • Provides patient specific medical record information to insurance companies in regards to severity of illness, intensity of service, discharge screens, concurrent services provided, and both routine and complex discharge planning issues in order to obtain authorization of services provided throughout the continuum of care.
  • Documentation in the medical record concise and relevant to the patient's program, plan of care, and discharge planning information.
  • Demonstrates self-motivation and initiative by developing and maintaining current information on community resources through attendance of continuing education and professional associations.
  • Displays non-judgmental acceptance, maintains confidentiality, and establishes a positive rapport and working relationship with patients/family/visitors/staff/peers/community.
  • Participates in leadership and collaboration with all members of the health care team for identification of opportunities for process improvement while maximizing efficient use of resources and minimizing delays in service.

Minimum Position Qualifications:

Education :

  • Bachelor's Degree in Social Work / Master's Degree in Social Work or Registered Nurse (BSN preferred for nursing staff)
  • Coursework/Training - Training and completion of clinical practicum within a medical social venue ascertaining human resource issues, leadership, process improvement, and social services processes

Experience :

  • 2 years of Clinical experience in Nursing, Utilization Review, Social Services, Discharge Planning, and/or Home Health.


  • Current license to practice nursing from the Texas State Board of Nurse Examiners, OR
  • Current Texas Social Work License with current certification to practice in the state of Texas.
