Responsibilities

Position Summary:

Community Wellness Coordinator will be responsible for:

  • Assisting in program management of Triple P, LiveWell Georgetown County, Community and Industry, Mobile Unit, grant initiatives, and any other areas of Community Wellness under Community Wellness Supervisor.
  • Responsible for assisting in financial management of budget items related to community outreach, education events, industry, etc.
  • Completion of ongoing data collection to support the program/grant goals, CHNA, and CBR. While ensuring identified needs are considered in the development of the strategic direction for the organization’s wellness initiatives.
  • Develop community dashboard and routinely share process and progress measures with partners.
  • Analysis of metrics, planning of countermeasures and monitoring of outcomes.
  • Providing health education and both chronic and acute disease management services to target populations.
  • Ensuring policy and procedures and standard work are developed and reviewed annually for all aspects of the community wellness Initiatives.
  • Assist in all program management, development, expansion of services, standardization, recruitment, communication, coaching, etc. for Triple P.
  • Engage potential or partner organizations into coalitions as appropriate.
  • Assisting in reporting for grant programs, as well as monthly dashboards
  • Participate in monthly learning collaboratives for grant programs
  • Foster and develop relationships with key community members to drive the development and implementation of the community action plan.
  • Facilitate purposeful meetings with a variety of partners and community members to achieve results and spur community action.

QUALIFICATIONS

Education:

  • Bachelor degree in social work, health education, community development, nonprofit management or other related field, required.
  • Master’s degree preferred.

Experience:

  • Three (3) or more years of experience in community outreach, community development/organizing, health education, social work, or nonprofit management required.
  • Previous experience in healthcare education setting and community coordination preferred.
  • Previous experience with coordination, leading, and managing community outreach and grant programming, preferred.
  • Prior experience in community health education and instruction, preferred.
  • Prior experience working with industries, preferred.
  • Minimum of 2 years of grant writing, management, and reporting, preferred

License/Certification:

Not required

Knowledge/Skills:

  • Excellent communication and consulting skills with a strong understanding of evaluation processes.
  • Ability to complete ongoing data collection to support grant goals and department outcomes per grants and system wide initiatives.
  • Experience in program development, program management, and implementation of new and existing programs.
  • Experience in leading groups.
  • An understanding of population-based, public health strategies to child maltreatment prevention.
  • Ability to work well in a team environment across multiple agencies, coalitions, and collaborative, overseeing compliance with set standards required.
  • Well organized and strong attention to detail and good computer skills.
  • Ability to write, implement and interpret administrative policies that reduce risk for the organization
  • Strong people skills which include clear written and verbal communication, ability to be collaborative, assertive and persuasive while maintaining positive and productive relationships.
  • Committed to understanding and conducting their work consistent with the mission, values and core principles of grant programming and community health resource initiatives.
  • Ability to proactively encourage community buy in related to the initiative goal(s)
  • Assist in coordination and management of fiscal manners related to grant and program budget items.
  • Ensuring development and completion of standard of works, policies, forms, training tools, program materials, outreach materials, etc.

Other:

  • Valid and in good standing South Carolina driver’s license required.
  • Personal transportation to be able to visit various “work locations” throughout the county required
  • Ability to work weekends and attend evening events as needed
