Job Description

We are looking for a Physical Therapist for the Physical Therapy Department at St. Mary Medical Center. St. Mary Medical Center is seeking an acute care PT per diem. If you love working with a great group of collaborative therapists, this is the site for you. Weekends and holidays required. Some weekday shifts may also be available.

Location: Apple Valley, CA

Work Schedule: Full Time - 80 Biweekly Hours

Shift: 8-hour, Days

Job Summary:

Evaluates, plans, directs and administers medically prescribed physical therapy programs in the hospital to restore function and prevent disability following disease or injury, or loss of body part. Assists patient to reach his/her maximum performance and assume a place in society while learning to live within the limits of his/her capabilities. The staff Physical Therapist also assumes responsibility and accountability for the care of those patients being treated by Physical Therapy Assistants and may be assigned to orient staff and students and to assume the duties and responsibilities of the supervisor in the absence of that individual. Performs administrative duties and participates in department Performance Improvement, Infection Control, Safety and Education programs

Essential Functions:

  • Provides patient/client care and assessment in accordance with established practice/departmental guidelines.
  • Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient populations served.
  • Develops an accurate individualized plan of care. Goals are feasible based upon findings and patient/family expectations.
  • Plans and makes appropriate discharge plans.
  • Evaluates effectiveness of treatment plan and revises as indicated.
  • Documents patient treatment, reports and charges on a daily basis.
  • Provides patient/family education and training according to established clinic/departmental guidelines.
  • Trains patient and/or primary care provider in post-discharge management.
  • Adapts conventional therapeutic techniques to meet the needs of patients unable to comprehend verbal commands or voluntarily carry out a regime of therapeutic exercises.
  • Plans and attends patient/family meetings, as appropriate events.
  • Prepares and presents appropriate material for educational workshops, lectures and training.
  • Assists with the training and supervision of subordinate staff, students and volunteers.
  • Provides feedback to Director/Manager regarding performance, as appropriate.
  • Supervises affiliating students according to department guidelines.

Skills:

  • Excellent oral and written communication skills.

Minimum Position Requirements:

Education:

  • Coursework/Training - Graduate of the American Physical Therapy Association training program.
  • Coursework/Training - Foreign trained applicants must complete successful entry level educational equivalency review.

Licenses/Certifications:

  • Current California Physical Therapy license.
  • Foreign Trained Physical Therapist - Current California Physical Therapy license within 9 months of hire date.
  • Current BCLS.

Preferred Position Qualifications:

Experience: Experience in an acute care or skilled nursing facility.
