Swedish Medical Center is looking for a Bone Marrow Transplant Coordinator (1.0 FTE, Day Shift) to work in Sci Hematology Oncology at Swedish First Hill campus in Seattle, WA.

The BMT Coordinator is responsible for assessment of needs, education, and coordination for stem cell transplant and CAR-T patients. The coordinator will communicate all aspects of the plan of care with patients, families, and other members of the Transplant Team, other health care providers, and payors.

In this position you will:

  • Monitors patient/family before, during and after transplantation for physical, emotional, psychosocial, and financial needs and intervenes when appropriate.
  • Coordinates the patient pre-admission work-up and visits. Gathers and reviews all pertinent clinical information (including past treatments and outcomes) and presents this to the transplant physician with special notes to abnormal results.
  • Collaborates with direct health care providers to establish plan of care, facilitates continuity of care across departments (both in-patient and out-patient) within hospital and home health providers.
  • Coordinates the transfer of patient care from Stem Cell Transplant Program back to referring physician and is responsible for long-term follow-up, which includes but is not limited to: periodic updates on patient condition, coordinating follow-up visits with Stem Cell Transplant physician and referring physician, and coordinating provision of post-discharge necessary services.
  • Educates patients, their families, to assist understanding of the disease, and processes and procedures surrounding pre- and post-transplant issues.

Qualifications:

Required Qualifications:

  • 3 years of current oncology nursing experience, preferably in an outpatient setting
  • Current Washington State Registered Nursing License
  • Basic Life Support (BLS) for Health Care Providers required at time of hire from American Heart Association (AHA) or Military Training Network (MTN)

Preferred Qualifications:

  • Bachelor's degree in Nursing from an accredited School of Nursing.
  • Applicants without a BSN degree must actively pursue and attain a BSN within six (6) years of hire.
  • Registered Nurses hired between January 1, 2013 and December 31, 2015 must actively pursue and attain a BSN within seven (7) years of hire. In the event that the employee does not acquire his/her BSN by the above deadlines, there will be a labor/management review process and discussion on the nurse’s individual circumstance.
  • Master's Degree in Nursing
  • Experience in Hematology / Stem Cell Transplantation
  • Oncology Certified Nurse certification
