Work Schedule: Full Time - 80 Biweekly Hours

Shift: Pay Per Visit Employee

Job Summary:

The physical therapy assistant assists in the provision of physical therapy service provided the service is rendered under the supervision of a physical therapist licensed by the board. Duties shall include following the physical therapist patient specific established treatment plan with the responsibility to report back to the supervising physical therapist any change in the patient's condition not consistent with planned progress or treatment goals.

Essential Functions:

  • Documents the full assessment and create a plan of care with accurate orders, diagnosis, and projection of visits for all disciplines necessary to support each patient in EHR.
  • Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.
  • Documentation completed and synchronized/submitted by 10:00 am the following day.
  • Conducts therapy practice within defined standards of care according to APTA guidelines and California State practice Act.
  • Follows the patient specific Plan of Care that has been established by the supervising physical therapist.
  • Provides direct patient care to meet the patient's functional needs to ensure optimal outcomes.
  • Notifies the supervising physical therapist of any change in the patient's condition not consistent with planned progress or treatment goals.
  • Assures continuation of therapeutic plan by instructing patients, families and caregivers in the home exercise programs as determined by the supervising physical therapist.
  • Makes visits as scheduled.
  • Contributes to patient case conferences to provide an interdisciplinary approach to the patient's care. Develops coordination and cooperation among other health care disciplines in the Agency to provide quality patient care.
  • Maintains patient rights and dignity.
  • Competes the patient care records and submits documentation timely and accurately according to network policy.
  • Communicates effectively with patients, families, and the home health team.
  • Enhances professional development through participation in educational programs, current literature, in-service meetings and workshops.

Minimum Position Requirements:

Education: Education to meet certification, license, or registration requirement.


  • Physical Therapy Assistant Certificate of Approval in State of California.
  • Current Basic Life Support certification.
  • Current California driver's license.
  • Proof of auto liability insurance.

Preferred Position Qualifications:

Experience: One (1) year in Home Health as a Physical Therapy Assistant.
