PALLIATIVE CARE COUNSELOR - ST. JOSEPH'S HOSPITAL NORTH(Job Number: 207494) Description   The Palliative Care Counselor provides skilled assessment and intervention for patients with chronic, complex and/or life-threatening conditions and their families, provides clinical consultation as an active member of the interdisciplinary palliative care team, advocates for interventions that alleviate burden and facilitate coping for palliative care patients and families, proficient in approaching end-of-life and ethical issues as well as bereavement/grief support for individuals and groups, facilitates transition for the patient throughout the palliative care admission, attends to dynamics that foster effective use of resources, service delivery and patient and family satisfaction, accountable to the palliative care team and leadership.
