In this role, you will be responsible for a range of functions
that include the preparation and dispensing of drugs in the outpatient setting,
maintenance and review of patient medication profiles, counseling patients on
appropriate drug use, and reviewing medication orders and patients medical
record. You will collaborate closely with other members of the health care team
to help prevent inappropriate or misuse of medications. Adhering to
departmental policies and procedures, you will deliver a wide range of specialty
pharmacy clinical services (e.g., assisting with prior authorization, meds to
beds, SAR, TAR processes as well as helping patients with financial assistance).

seeking a dedicated clinical professional with:
Pharm D. degree and current CA Pharmacy license (required).
pharmacy experience (highly desired).

Knowledge of and ability to perform age specific and disease specific
