The Certified Diabetes Educator (CDE) for BayCare Medical Group provides a direct professional contribution to the care and self-management of people with diabetes. The CDE provides patient-centered diabetes self-management training, education and support to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators (AADE), and is an expert resource for people and providers. The CDE may be involved in case management, clinical management, healthcare consultancy with other providers, organizations, industry; public and professional education; public health and wellness promotion; research in diabetes management and education; and may have responsibilities supporting the recognized program meeting requirements set forth by the Centers for Medicare & Medicaid Services accrediting process. Performs other duties as assigned.
Required Experience includes 3 years as a Certified Diabetes Educator; 1 year experience with DSMT/E/S program preferred.
