PATIENT NAVIGATOR - REGISTERED RESPIRATORY

The Patient Navigator - Registered Respiratory is responsible to provide support, education, referral recommendations, coordination of care, and other assistance to the patient and their family from diagnosis and treatment to recovery. Provide comprehensive education to patients diagnosed with chronic obstructive pulmonary disease (COPD). This education will include normal lung anatomy and physiology, physiologic changes caused by COPD, medications typical for COPD including what, when, how and why they take them, recognizing exacerbation vs. bad day, pulmonary rehabilitation and exercise, smoking cessation and oxygen usage. Utilize written educational materials along with educational aides that include; handouts, lung models and videos as deemed appropriate. When applicable, recommend referrals for pulmonary rehabilitation and pulmonary function testing (PFTs). Act as liaison to physicians and discharge planners to ensure proper length of stay, adequate education and safe discharge. Required Certifications include Completion of BayCare online Case Management Internship modules within 6 months and Credentialed as a COPD Educator via the AARC course within 6 months; in addition to the Basic Life Support certification.
